The 'Active Life'
programme is already running for
Practices in the Staveley area but
will be available for Chesterfield
Practices from March 2008.
The scheme is run
by Chesterfield Borough Council and
Derbyshire County PCT Health
Promotion Service.
It aims to
increase the physical activity
levels in adults who are mainly
sedentary or inactive and who are
living with, or at high risk of
developing long term health
conditions. This includes
diabetes.
If you are
motivated to lose weight and improve
your fitness levels please contact
your GP or Practice Nurse for more
information and to discuss referral.
The activity
sessions are held in the new Healthy
Living Centre at Staveley and there
is some support towards the cost.

Join our
'Active Life' Scheme
In Chesterfield we
recognise that a large
number of people are not
sufficiently active
to benefit their health and that a
wide
range of medical conditions can be
improved or prevented through
exercise.
For those advised by a medical
practitioner
to exercise the Active Life scheme
provides
additional support and advice within
a
supervised environment.
Initially a
consultation will be arranged with a
qualified Referral Programme
Consultant who will devise an
appropriate exercise routine.
Ongoing support continues during
specified times throughout the
ten-week scheme.
We provide:
- A
confidential service
- One-to-one
consultation and ongoing support
- Liaison with
your referrer
(GP, physio, dietician etc.)
- Understanding
your condition and any
difficulties you may face
- A tailored
exercise programme that you will
enjoy
- An
environment that is friendly,
caring and comfortable
- Support and
help to achieve your goals
- Contact with
others who have experienced
similar problems
Our trained
Fitness Consultants provide health
programmes at the following session
times:
Monday to
Friday 9.30am to 5.00pm
Saturday 11.00am to 2.00pm
