'DIABETES AND YOU' is an educational course for all people who have been recently diagnosed with Type 2 Diabetes.  It consists of 3 two hour sessions and covers many aspects of diabetes including:

ü What is Diabetes?
ü Medication and management
ü Lifestyle choices e.g. increasing activity, quitting smoking
ü Healthy eating - discussion with the dietician
ü Foot Care - discussion with the podiatrist
ü Minimising the complications of diabetes

It offers the chance to meet and chat with others who have diabetes and you are welcome to bring a relative or friend to the meetings.

The sessions for our surgery are held locally in Chesterfield.

From the 1st January 2008 we will automatically refer anyone who is newly diagnosed with Type 2 Diabetes.  You can decline if you wish but we would like to encourage everyone to take advantage of these informative, friendly meetings.

If you have been diagnosed recently (but before January 2008) and would like to be referred please contact one of the Diabetes Nurses at Avenue House.  Even if you have been diagnosed a while but feel you would benefit from attending a course, please chat with us about being referred.

Living with any long term condition may affect your everyday life. If you feel you would like to develop your skills and to change things for the better, then the ‘Living With Long Term Conditions’ (LWLTC) course may be for you. Click here to download a leaflet which explains more.
