Your Annual
Diabetes Review
We are
changing your annual diabetes review
(usually done in your birthday
month) to give you the opportunity
to feel more involved in your
diabetes care.
We are encouraging
you to have your results before you
come to see the Diabetes Nurse for
your review. At the moment,
results will be sent to you by post
but we can consider other options as
the service develops e.g. email, or
by calling into the surgery.
We hope that, by
having the information in advance,
this will give you the chance to
think about what your results mean
to you and what changes you think
will help you in living with
diabetes. This will then form
the basis of your review with the
Diabetes Nurse.

The system will
hopefully work as follows:
- You will see
a Health Care Professional on the
same day that you have your
blood test. (You can book this
appointment up to one month in
advance.)
- During this
appointment:
- blood
pressure is checked (or we
can record your home
readings)
- an early
morning urine sample is
dipped and sent off
- lifestyle
information is updated
- At the moment, we will send your blood results to you by post. They will form part of your Care Plan Record. This will give you time to think about your results and what goals are important to you.
- You then
bring your care plan with you
when you see the Diabetes Nurse
to discuss your goal/s for the
year. This gives you the
opportunity to discuss what is
important to you and we can
offer information as needed.
Interim reviews can be used to
see how you are progressing with
your goals and to update your
diabetes blood tests.
Care Planning
is a good opportunity for you to
become more involved in deciding how
to manage your diabetes.
